For the 5 Ultimate 100 Camps in the East, Southeast, Southwest, West, and Midwest you must attend the National Underclassmen Combines to qualify. For the Top Prospect Camp you must fill out this form. You can automatically qualify by winning an MVP at the U100 Camp or NUC 5 Star. You may also nominate yourself here.
